Winery tour and Dinner
You are cordially invited to a winery tour and dinner at Chateau des Charmes, 1025 York Road,
on Saturday, May 5 at 6:00 pm. If you would like to join us for this event, the cost is $90 per
person and you can sign up on the registration page. The cut-off date for signing up for this
event is April 20, 2007. Due to financial commitment with Chateau des Charmes, no refunds
will be issued after April 20. Space is limited to 50 registrants.

Course description and objectives
This will be the first annual scientific meeting of the Canadian Neuromodulation Society. The
program will focus on new development of neuromodulation techniques for pain and
neurological diseases. There will be two keynote lectures, one on future of neuromodulation and
one on review of mechanisms of neuromodulation in pain. There will be an oral presentation for
scientific papers and Annual General Meeting as well. Participants will be able to identify and
assess the suitable candidate to expand the indications of various neuromodulation devices; will
be able to understand the various mechanisms of neuromodulation pain management; will be
able to stratify their pharmacologic options in patients with neuropathic pain.
Accreditation
This event is an Accredited Group Learning Activity (Section 1) as defined by the Maintenance
of Certification program of The Royal College of Physicians and Surgeons of Canada, approved
by University of Toronto (9.5 credit hours). The Continuing Medical Education Office of the
University of Toronto designates this educational activity for a maximum of 9.5 credit hours in
Category 1 of the Physician’s Recognition Award of the American Medical Association. Each
physician should claim only those hours of credit that he/she actually spent in the educational
activity.
Accommodation
A block of rooms has been reserved at the White Oaks at a special conference rate of $185.00 for
single or $195.00 double occupancy. Reservations: You may either register on-line
(instructions below) or if you do not have computer access, a registration form will be sent to
you by fax or mail (at your request)
so that you can mail or fax to White Oaks to reserve a
room. The cut-off date for reservations is April 4, 2007. There is no guarantee that the room rate
of $185.00/$195.00 will still apply or that a room will be available after April 4, 2007.
