I Nurse Practitioner - Practice Nurse - Health Care Assistant- Administration Team 4 ROLE OF THE PRACTICE NURSE- Responsibility for the delivery of general practice nursing to the whole practice population. The focus of the role is both the delivery of evidence-based practice for patients presenting with a long term condition and the provision of preventative health care to the practice population. Responsible for demonstrating critical thinking and skills in clinical decision making in the management of the patients. Work collaboratively with the practice team to meet the needs of patients, prepare, update and deliver policy and protocols, as required. 5. KEY AREAS OF RESPONSIBILITY: KEY RESULT AREAS / MAIN TASKS
Health promotion in chronic disease management eg CHD diabetes, asthma, stroke, hypertension etc as
Cervical cytology Travel vaccine information and administration
Cryotherapy To take an active role in preparation/updating of protocols of work in related areas
To assist in minor surgery and IUCD insertion
To ensure information is properly recorded in IT systems Act as clinical leader in delivery of GPN services to patients ensuring needs of patients are the priority
Participate in planning and implementing changes in areas of care and responsibility Take responsibility for own development learning and performance and undertake clinical supervision as
Support team members to explore and develop new and efficient ways of working where appropriate Mentor/coach staff on clinical matters
6. ASSIGNMENT, GENERATION AND REVIEW OF WORK
Work is generally generated by: invitation letters self-referral by patients referrals from GPs at the end of each session or day, there are allocated slots to ensure that work is gone over if required with the duty doctor eg changes or proposed changes of medications, possible referrals to hospital, any patient or work issues arising
7. COMMUNICATIONS AND WORKING RELATIONSHIPS
Communications will include GPs, fellow practice nurses, other healthcare professionals, other clinical staff outwith the practice, admin staff within and outwith the practice and patients these communications will take the form of e-mail, verbal (phone/face-to-face), written
8. QUALIFICATIONS AND/OR EXPERIENCE SPECIFIED FOR THE POST – if there are training requirements for the successful candidate these will be addressed to ensure a balance within the team. The structure of the practice team is that we would expect each nurse in the team to provide cervical cytology, CDM, travel immunisations and for each nurse then to specialise in asthma and/or diabetes to ensure full delivery to patients. There are other tasks and duties which are required of the post-holder eg spirometry, ECGs, venepuncture, depo-provera, well person and new patient checks, cryotherapy. The post is NOT part of the community nursing team and the post holder will not therefore be required to undertake routine dressings etc. 9 REPORTING – the practice manager will be the line manager of the post holder. Clinical matters should be referred / discussed with either Nursing team colleague or GP.
